Suzanne Hughes: I know you have a very strong background as a nurse practitioner in the field of preventive cardiology. For the last several years along with working in a clinical setting you have worked in a research environment studying insulin resistance. How is this work different for you? And what have you learned in studying insulin resistance that is applicable to the field of cardiovascular risk reduction? Cindy Lamendola: Well even in clinical research there is always an opportunity for patient education and now my focus not only includes cardiovascular risk factors and prevention but also insulin resistance prediabetes and the different mechanisms that lead up to cardiovascular disease and type 2 diabetes and other clinical outcomes. The best part of being involved in research is just that—there are so many opportunities to look at mechanisms that influence different clinical outcomes. In the past 12 years of working in research on insulin resistance we have seen the consequences of insulin resistance grow from prediabetes diabetes and cardiovascular risk factors that increase risk for cardiovascular disease to now include: polycystic ovary syndrome nonalcoholic fatty liver disease obstructive sleep apnea and schizophrenia as well as other possible outcomes such as some type of cancers and cardiomyopathy. We now know that when we see patients with these other medical problems they may also be at increased risk for type 2 diabetes and heart disease. 1SH: How exactly is insulin resistance measured?CL: Currently the gold standard is by doing a test called a Euglycemic CLAMP or an insulin sensitivity test.
